Schedule an Appointment
For your convenience at Alvernon Optical you can book an appointment online right from this website. Simply select the location, the date and appointment time you’d like, and fill out the requested information.
Click “submit” and our office will call you back with confirmation that the appointment has been scheduled.
Be sure to provide the best call-back number where we can reach you during business hours… We may need to get additional personal or insurance information before your appointment.
Next day appointments made for times before 11:00am must be made prior to 5:00pm the night before. Please don’t arrive to your appointment without a confirmation call. All Saturday appointments must be made by telephone.

As an added convenience, you can print out all the necessary paperwork right from this website before your eye appointment…
Simply click on the ‘forms’ button, and be sure to bring all the completed forms, along with your insurance ID with you to your eye exam.
